Terms and Conditions:

I hereby release Set'em Up, employees/independent contractors from all liability for personal injury, illness, or property damage occurring on or off the premises. I authorize Set'em Up to see medical treatment at the nearest medical facility and call paramedics and discharge my player to an ambulance if I am not able to authorize in case of a medical emergency. I certify that my player is in good health and capable of participating in physical activities. I hereby give permission to Set'em Up to take and use photographs for promotional uses.  I herby allow Set'em Up to contact me with text messages with updates as I have provided phone number. I hereby authorize Set'em Up to charge my card monthly for auto renewal plans. I understand that payment is non refundable.
